ISRO Begins Detailed Review After PSLV Launch Failure
Chennai — India’s space agency, the Indian Space Research Organisation (ISRO), has launched a comprehensive investigation after its PSLV-C62 mission failed due to a malfunction in the rocket’s third stage. The mission was carrying a satellite payload that was lost when the rocket veered off course shortly after stage separation.

ISRO officials said telemetry data is being analysed frame by frame to determine what triggered the deviation. A failure analysis committee has been formed, bringing together propulsion, navigation, and quality-control experts to examine every phase of the flight.

The PSLV is known as one of the world’s most reliable launch vehicles, and the incident comes as a setback for India’s growing commercial satellite-launch business. While no human life was at risk, the failure is expected to delay upcoming missions until corrective measures are implemented.

Officials stressed that transparency and technical learning would be the focus moving forward. ISRO has assured international clients that reliability remains a priority.
